ENROLMENT FORM

INSTRUCTIONS:

  • Your first invoice will include a £20 Joining Fee which includes a WHAM T-Shirt.  

  • You will be required to make an initial payment for the rest of the half term. After this, we will take further payments every half term automatically (similar to a Gym Membership) 

  • Your bank will need to support open banking to make the payment. If it doesn't, please email us at info@whamtheatreschools.com for an alternative form.

  • Please fill out all of your details below and complete every box

  • If you're enrolling more than one child, click "add student" button

  • Our memberships are half termly and you can end your membership by providing notice by the last session at the end of the current half term. Please click here to see our term dates.

  • Once enrolled, your Direct Debit payment will be automatically activated & your first (non-refundable) payment will be generated.

  • The direct debit is set up to automatically collect your fees, this is our required payment method. 

  • Classes can only begin for your child once the enrolment form & direct debit have been set up
